aria-level=\"1\"><span style=\"font-weight: 400;\">The year <\/span><b>2024 commemorates 50 years s<\/b><span style=\"font-weight: 400;\">ince the launch of the<\/span><b> Expanded Programme on Immunization (EPI) <\/b><span style=\"font-weight: 400;\">by the <\/span><b>World Health Organization (WHO) in 1974.<\/b><\/li>\n<\/ul>\n<h2><span style=\"font-size: 18pt;\"><b>About Expanded Programme on Immunization (EPI):<\/b><\/span><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Launch<\/b><span style=\"font-weight: 400;\">: EPI was launched in<\/span><b> 1974 to ensure that all children<\/b><span style=\"font-weight: 400;\">, in all countries, benefited from <\/span><b>life-saving vaccines.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Smallpox Eradication:<\/b><span style=\"font-weight: 400;\"> EPI was introduced as the<\/span><b> eradication of smallpox virus w<\/b><span style=\"font-weight: 400;\">as on the horizon.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-size: 18pt;\"><b>Global and National Immunization Progress:<\/b><\/span><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Expanded Vaccine Portfolio:<\/b><span style=\"font-weight: 400;\"> While in 1974, there were<\/span><b> vaccines to prevent six diseases,<\/b><span style=\"font-weight: 400;\"> five decades later, there are <\/span><b>vaccines against 13 diseases <\/b><span style=\"font-weight: 400;\">which are<\/span><b> universally recommended; <\/b><span style=\"font-weight: 400;\">and <\/span><b>vaccines against 17 additional diseases <\/b><span style=\"font-weight: 400;\">are recommended for a<\/span><b> context-specific situation.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Increased Coverage: <\/b><span style=\"font-weight: 400;\">Significant rise in vaccination coverage rates globally, with <\/span><b>DPT3 coverage <\/b><span style=\"font-weight: 400;\">increasing from<\/span><b> 5% in the early 1970s to 84% by 2022.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>India\u2019s Immunization Journey: <\/b><span style=\"font-weight: 400;\">Launch of <\/span><b>EPI in 1978, r<\/b><span style=\"font-weight: 400;\">enamed to<\/span><b> Universal Immunization Programm (UIP) in 1985.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Success of EPI: <\/b><span style=\"font-weight: 400;\">S<\/span><b>mallpox has been eradicated, Polio e<\/b><span style=\"font-weight: 400;\">liminated from all but two countries and many <\/span><b>vaccine preventable diseases <\/b><span style=\"font-weight: 400;\">have n<\/span><b>early disappeared.\u00a0<\/b><\/li>\n<\/ul>\n<h2><span style=\"font-size: 18pt;\"><b>Impact of EPI:<\/b><\/span><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Lives Saved<\/b><span style=\"font-weight: 400;\">: Immunization programs <\/span><b>credited with saving millions of lives<\/b><span style=\"font-weight: 400;\">, preventing billions of hospitalizations, and <\/span><b>curbing diseases like smallpox and polio.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Cost-Effectiveness: <\/b><span style=\"font-weight: 400;\">Economic analyses show vaccines to be <\/span><b>highly cost-effective, <\/b><span style=\"font-weight: 400;\">with a <\/span><b>seven to eleven-fold return <\/b><span style=\"font-weight: 400;\">on investment for<\/span><b> every dollar spent.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Success of Immunization Program<\/b><span style=\"font-weight: 400;\">: In nearly all<\/span><b> low- and middle-income countries<\/b><span style=\"font-weight: 400;\">, including India, the <\/span><b>immunization programme <\/b><span style=\"font-weight: 400;\">remains a success among<\/span><b> all government initiatives.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Immunization in Mixed Health Systems:<\/b><span style=\"font-weight: 400;\"> In<\/span><b> mixed health systems <\/b><span style=\"font-weight: 400;\">with both the<\/span><b> public and private sector <\/b><span style=\"font-weight: 400;\">delivering services.<\/span><b> Immunization <\/b><span style=\"font-weight: 400;\">remains the <\/span><b>only health intervention <\/b><span style=\"font-weight: 400;\">with <\/span><b>greater utilization f<\/b><span style=\"font-weight: 400;\">rom the <\/span><b>government sector.<\/b>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">In India, the share of the private sector in overall<\/span><b> health services is nearly two thirds; Nearly 85% to 90% <\/b><span style=\"font-weight: 400;\">of all vaccines are<\/span><b> delivered from government facilities.<\/b><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h2><span style=\"font-size: 18pt;\"><b>UNICEF\u2019s \u2018The State of the World\u2019s Children\u2019 report(2023):<\/b><\/span><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Childhood Immunization Decline<\/b><span style=\"font-weight: 400;\">:The childhood i<\/span><b>mmunization coverage had declined in 2021<\/b><span style=\"font-weight: 400;\">.<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><b>\u00a0In 2022,<\/b><span style=\"font-weight: 400;\"> globally, an<\/span><b> estimated 3 million children <\/b><span style=\"font-weight: 400;\">were<\/span><b> zero dose (did not receive <\/b><span style=\"font-weight: 400;\">any recommended vaccine).\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">A<\/span><b>nother 6.2 million c<\/b><span style=\"font-weight: 400;\">hildren were partially immunized.<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Improving Vaccination Coverage in India: <\/b><span style=\"font-weight: 400;\">Over the years, the <\/span><b>vaccination coverage in India has increased, <\/b><span style=\"font-weight: 400;\">both nationally and State-wise.\u00a0<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">There are <\/span><b>persisting inequities in coverage by geography, socio-economic<\/b><span style=\"font-weight: 400;\"> strata and other parameters, which<\/span><b> demand urgent interventions.<\/b><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<h2><span style=\"font-size: 18pt;\"><b>Background of Vaccine development:<\/b><\/span><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>First Vaccine Development: <\/b><span style=\"font-weight: 400;\">The first available vaccine was <\/span><b>against smallpox in 1798.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Early Vaccine Development: <\/b><span style=\"font-weight: 400;\">The first a<\/span><b>nti rabies vaccine, cholera, and typhoid vaccines <\/b><span style=\"font-weight: 400;\">developed between 1<\/span><b>880s to mid 1890s <\/b><span style=\"font-weight: 400;\">were primarily for adults.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>India&#8217;s First Plague Vaccine:<\/b><span style=\"font-weight: 400;\"> The<\/span><b> first vaccine ever developed<\/b><span style=\"font-weight: 400;\"> in any part of the world against <\/span><b>plague (in 1897) was from India<\/b><span style=\"font-weight: 400;\"> and meant for <\/span><b>individuals across all age groups.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Introduction of BCG Vaccine for Tuberculosis:<\/b><span style=\"font-weight: 400;\"> The<\/span><b> BCG vaccine (against tuberculosis) <\/b><span style=\"font-weight: 400;\">was first introduced in a <\/span><b>nationwide campaign in 1951.<\/b><span style=\"font-weight: 400;\"> It was <\/span><b>administered to the adult population.<\/b><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Influenza Vaccines: I<\/b><span style=\"font-weight: 400;\">nfluenza vaccines have been<\/span><b> administered to adults and children alike.<\/b><\/li>\n<\/ul>\n<h2><span style=\"font-size: 18pt;\"><b>Strategies for Adult Immunization:<\/b><\/span><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Policy Expansion: <\/b><span style=\"font-weight: 400;\">Expanding immunization <\/span><b>coverage in additional populations<\/b><span style=\"font-weight: 400;\">: The recent announcement on <\/span><b>HPV vaccines for teenage girls is a good start.<\/b>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">The Indian government needs to<\/span><b> consider providing recommended vaccines f<\/b><span style=\"font-weight: 400;\">or a wider section of the <\/span><b>adult and elderly population.<\/b><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Recommendation Framework: <\/b><span style=\"font-weight: 400;\">Considering that vaccines are <\/span><b>highly cost effective, o<\/b><span style=\"font-weight: 400;\">nce recommended by the <\/span><b>National Technical Advisory Group on Immunization (NTAGI).\u00a0<\/b>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Vaccines for all <\/span><b>age groups should be made available <\/b><span style=\"font-weight: 400;\">as f<\/span><b>ree at the government <\/b><span style=\"font-weight: 400;\">facilities.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">NTAGI should start p<\/span><b>roviding recommendations on the use of vaccines<\/b><span style=\"font-weight: 400;\"> in adults and the elderly.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Once a vaccine is <\/span><b>recommended by the government body<\/b><span style=\"font-weight: 400;\">, the<\/span><b> coverage is likely to be far greater <\/b><span style=\"font-weight: 400;\">than if the vaccines are <\/span><b>not recommended by the government.<\/b><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Addressing Vaccine Hesitancy: <\/b><span style=\"font-weight: 400;\">Prevailing <\/span><b>myths and misconceptions abo<\/b><span style=\"font-weight: 400;\">ut vaccines must be proactively addressed to t<\/span><b>ackle vaccine hesitancy.<\/b>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">The government must consider the help of <\/span><b>professional communication agencies <\/b><span style=\"font-weight: 400;\">to <\/span><b>dispel myths <\/b><span style=\"font-weight: 400;\">(and in a layperson\u2019s language and with the use of social media).\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">This requires citizens to <\/span><b>learn and educate themselves about these vaccines <\/b><span style=\"font-weight: 400;\">from <\/span><b>reliable sources.<\/b><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Physician Engagement: <\/b><span style=\"font-weight: 400;\">Various professional associations of <\/span><b>doctors \u2014 community medicine experts, family physicians and pediatricians <\/b><span style=\"font-weight: 400;\">should work to<\/span><b> increase awareness about vaccines<\/b><span style=\"font-weight: 400;\"> among adults and the elderly.<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Physicians treating patients with any <\/span><b>disease should use the opportunity to make t<\/b><span style=\"font-weight: 400;\">hem <\/span><b>aware of vaccines.<\/b><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Research and Evidence Generation: Medical colleges and research institutions<\/b><span style=\"font-weight: 400;\"> should generate evidence on the <\/span><b>burden of diseases in the adult population in India.<\/b><\/li>\n<\/ul>\n<h2><span style=\"font-size: 18pt;\"><b>Way Forward:<\/b><\/span><\/h2>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Integration and Expansion:<\/b><span style=\"font-weight: 400;\"> The introduction of n<\/span><b>ew vaccines in national programmes <\/b><span style=\"font-weight: 400;\">contributes to i<\/span><b>ncreased coverage of all existing vaccines.\u00a0<\/b>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Expanding coverage of v<\/span><b>accines for adults and the elderly <\/b><span style=\"font-weight: 400;\">may result in improved coverage with <\/span><b>childhood vaccines and reduced vaccine<\/b><span style=\"font-weight: 400;\"> inequities.<\/span><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Advancing EPI<\/b><span style=\"font-weight: 400;\">: India\u2019s EPI has made <\/span><b>major progress and it is time for another independent <\/b><span style=\"font-weight: 400;\">national level <\/span><b>review of the UIP in India, <\/b><span style=\"font-weight: 400;\">engaging<\/span><b> key partners and international experts.<\/b>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><b>In late 2023,<\/b><span style=\"font-weight: 400;\"> India launched a<\/span><b> pilot initiative of adult BCG vaccination <\/b><span style=\"font-weight: 400;\">as part of efforts to <\/span><b>\u2018end TB\u2019 from India.<\/b><\/li>\n<\/ul>\n<\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><b>Enhancing Awareness<\/b><span style=\"font-weight: 400;\">: The<\/span><b> COVID-19 vaccination of the adult population ha<\/b><span style=\"font-weight: 400;\">s made the public sensitized to the n<\/span><b>eed for and the benefits of adult vaccinatio<\/b><span style=\"font-weight: 400;\">n.<\/span><\/li>\n<\/ul>\n<h2><span style=\"font-size: 18pt;\"><b>Conclusion:<\/b><\/span><\/h2>\n<p><span style=\"font-weight: 400;\">In the <\/span><b>50 years of the EPI, it<\/b><span style=\"font-weight: 400;\"> is time for<\/span><b> another expansion of the programme<\/b><span style=\"font-weight: 400;\"> with focus on zero dose children, <\/span><b>addressing inequities in vaccine coverage <\/b><span style=\"font-weight: 400;\">and <\/span><b>offering vaccines to adults and the elderly<\/b><span style=\"font-weight: 400;\">.
