in constructing the Airport, the Government shall comply with all applicable laws and also with environmental norms.<\/span><\/p>\n<p><b>No lessons learned:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The government agencies did not comply with laws or norms, and several violations led to the <\/span><b>loss of 158 lives on May 22, 2010, when AIE 812 crashed on landing in Mangalore.\u00a0<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The <\/span><b>cover-up report,<\/b><span style=\"font-weight: 400;\"> which contained a well-orchestrated litany of lies, resulted in lessons not being learned to prevent a recurrence.<\/span><\/li>\n<\/ul>\n<p><b>The cardinal principle of an accident investigation:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">It is to <\/span><b>identify the cause and prevent recurrence.\u00a0<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">India has had a string of runway overruns due to <\/span><b>high and fast approaches and late touchdowns,<\/b><span style=\"font-weight: 400;\"> resulting in hull losses.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">In various shocking accidents, the <\/span><b>pilots were blamed<\/b><span style=\"font-weight: 400;\"> but there was <\/span><b>no introspection<\/b><span style=\"font-weight: 400;\"> on why investigations had failed to prevent such accidents from recurring, <\/span><b>indicating a lack of accountability and transparency in the system.\u00a0<\/b><\/li>\n<\/ul>\n<p><b>The Civil Aviation Safety Advisory Council (CASAC):<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The Ministry of Civil Aviation constituted the Civil Aviation Safety Advisory Council (CASAC) <\/span><b>after the Mangalore crash.\u00a0<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Members of operations, aerodromes and Air Traffic Control (ATC) were <\/span><b>tasked with identifying deficiencies and providing corrective steps.\u00a0<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The CASAC pointed out the <\/span><b>failure of the court of inquiry report<\/b><span style=\"font-weight: 400;\"> in identifying serious errors and taking corrective steps.\u00a0<\/span><\/li>\n<\/ul>\n<p><b>Warnings by CASAC:<\/b><\/p>\n<ul>\n<li><span style=\"font-weight: 400;\">The CASAC warned the Ministry that the failure to provide the <\/span><b>mandatory Runway End Safety Areas (RESA) at Calicut<\/b><span style=\"font-weight: 400;\"> could result in fatal accidents on Runway 10 if any aircraft touched down late at high speed.\u00a0<\/span><\/li>\n<li><span style=\"font-weight: 400;\">The <\/span><b>narrow service road<\/b><span style=\"font-weight: 400;\"> would prevent quick rescue by rescue and firefighting vehicles.\u00a0<\/span>\n<ul>\n<li><b>But the Ministry and the DGCA ignored our warnings.\u00a0<\/b><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><b>Issues:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">When the same errors cause accidents over years, it means that there are <\/span><b>serious deficiencies in training, problems with the safety audits by the DGCA.<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">The investigation agencies have <\/span><b>failed in identifying the reasons<\/b><span style=\"font-weight: 400;\"> for such accidents and providing corrective steps.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Our airports are <\/span><b>not conforming to international safety standards.\u00a0<\/b><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">There is <\/span><b>no transparency or accountability.\u00a0<\/b><\/li>\n<li style=\"font-weight: 400;\"><b>Fatigue,<\/b><span style=\"font-weight: 400;\"> another serious safety issue, is not given the importance it deserves.<\/span><\/li>\n<\/ul>\n<p><b>Conclusion:<\/b><\/p>\n<p><span style=\"font-weight: 400;\">Unless <\/span><b>occupational safety and training<\/b><span style=\"font-weight: 400;\"> are given top priority, the skies are not going to be
