Childhood Obesity in India: Rising Crisis Amid Double Burden of Malnutrition

India is witnessing a double burden of malnutrition – persistent undernutrition alongside a rapid rise in childhood overweight and obesity. 

  • The World Obesity Atlas, released by the World Obesity Federation, estimates that more than 40 million children in India are either overweight or obese.

About Childhood Obesity

  • Childhood obesity is a medical condition in which a child accumulates excess body fat that negatively affects health and well-being. It is generally assessed using the Body Mass Index (BMI) for age, where a child’s weight is compared with standardized growth charts.
  • According to the World Health Organization, a child is considered overweight or obese when their BMI-for-age is significantly above the healthy range for their age and sex.

Challenges

  • Poor Dietary Quality: Children at both extremes of the nutritional spectrum often lack proteins, vitamins, and essential minerals.
    • This results in hidden hunger (micronutrient deficiency) even among overweight children.
  • Rapid Nutrition Transition: Rising urbanisation, sedentary lifestyles, and access to processed foods are driving childhood obesity.
    • Unlike developed countries where dietary shifts occurred gradually, India has experienced this transition within the last 15 years, making the health system less prepared.
  • Dual Burden of Malnutrition: A significant share of Indian children do not consume adequate calories for healthy growth.
    • At the same time, many children consume diets high in sugar, refined carbohydrates, salt, and unhealthy fats.

Causes of Rising Childhood Obesity

  • Sedentary Lifestyle: Increased screen time and reduced physical activity among children.
  • Urbanisation and Processed Foods: Easy availability of cheap, calorie-dense processed foods.
  • Limited Nutrition Awareness: Lack of reliable information about healthy diets and recommended sugar intake.
  • Intergenerational Nutritional Deficits:  A survey by UNICEF found that nutritional deficits among children are often a carryover of the poor dietary habits of their mothers.
  • Socioeconomic Factors: A  2020 study published in Indian Paediatrics, suggests that obesity is no longer confined to affluent households — children in families with limited access to healthy food options may rely on relatively inexpensive, calorie-dense processed foods. 

Implications

  • According to the WHO, excess body weight during childhood significantly increases the risk of conditions such as type 2 diabetes and cardiovascular diseases later in life. 
    • The coexistence of undernutrition and obesity could, therefore, aggravate India’s already formidable non-communicable disease challenge.

Government Initiatives

  • POSHAN Abhiyan: It is a flagship programme to reduce malnutrition among children, pregnant women, and lactating mothers with a focus largely on calorie intake and nutritional supplementation.
  • School-Based Awareness: The Central Board of Secondary Education (CBSE) directed schools to display information on recommended sugar intake, promoting healthy dietary awareness.

Way Forward

  • Stronger Regulation of Junk Food: The government should implement stricter regulations on junk-food advertising, marketing, and availability, particularly around schools.
  • Women’s Empowerment and Maternal Nutrition: Children’s dietary patterns are closely linked to the nutritional awareness and health of mothers.
    • Improving women’s access to education, healthcare, and nutritional information can significantly enhance household dietary habits.
  • Integrated Public Health Strategy: Tackling childhood obesity requires a coordinated approach involving health, education, urban development, and food regulation sectors.
