Supreme Court Examines Mandatory Nucleic Acid Test (NAT) for Blood Safety in India

The Supreme Court has decided to look into whether blood banks should compulsorily conduct the Nucleic Acid Test (NAT) for identification of diseases.

  • A Bench led by Surya Kant, with Justices Joymalya Bagchi and Vipul M. Pancholi, questioned the cost-effectiveness of Nucleic Acid Testing (NAT) compared to the commonly used Enzyme-Linked Immunosorbent Assay.
  • The Court asked the petitioner to provide data on:
    • States where NAT is currently used
    • Number of hospitals implementing it
    • Cost implications for States
  • Right to Safe Blood as a Fundamental Right: The petitioner NGO argued that safe blood transfusion is part of the Right to Life under Article 21 of the Constitution of India.

About Nucleic Acid Test (NAT) 

  • NAT is a molecular diagnostic technique used to detect the genetic material (DNA or RNA) of pathogens in blood or other biological samples.
  • It can identify infections such as:
    • Human Immunodeficiency Virus
    • Hepatitis B
    • Hepatitis C
  • It helps identify infections at a very early stage, even before antibodies develop.

Working Principle

  • Principle:  NAT detects the genetic material (DNA/RNA) of pathogens directly in a sample, enabling early detection of infections even during the window period.

  • Working Mechanism
    • Sample Collection: Blood or body fluid is collected.
    • Nucleic Acid Extraction: Viral DNA/RNA is isolated from the sample.
    • Amplification: The genetic material is multiplied using techniques like Polymerase Chain Reaction (PCR).
    • Detection: Specific probes identify pathogen sequences (e.g., HIV, HBV, HCV).
    • Result: Presence of viral nucleic acid confirms infection.

Advantages of Nucleic Acid Test

  • Improved Blood Transfusion Safety: NAT significantly reduces the risk of transfusion-transmitted infections (TTIs).
    • For Example: Screening donated blood for Hepatitis B, HIV, and Hepatitis C before transfusion.
  • Crucial for High-Risk Patients: Patients requiring frequent blood transfusions benefit greatly from NAT screening.
    • For Example: Individuals suffering from Thalassemia often undergo repeated transfusions and are vulnerable to infected blood.
  • Strengthens Public Health Surveillance: NAT helps in early identification of viral outbreaks and infection patterns.
    • For Example: Early detection of emerging infections during blood screening programs.
  • Higher Sensitivity and Accuracy: NAT has greater sensitivity and specificity as it directly identifies the pathogen rather than the immune response.
    • For Example: Detection of Hepatitis C infections that may remain undetected in early stages through ELISA.

Limitations

  • Requirement of Skilled Personnel: The test involves complex molecular techniques such as Polymerase Chain Reaction, which require trained laboratory professionals.
    • Lack of skilled manpower can limit widespread adoption
  • Limited Availability in Rural Areas: Most advanced diagnostic laboratories are concentrated in urban centres, making NAT less accessible in rural or smaller hospitals.
    • For Example: Several district-level blood banks in India still rely primarily on ELISA.
  • Operational and Maintenance Challenges: NAT equipment requires strict laboratory conditions, regular calibration, and quality control.
    • Any technical error can affect the accuracy of results.
  • High Cost of Implementation: NAT requires expensive equipment, reagents, and advanced laboratory infrastructure.
    • For Example: Many government blood banks in developing regions may struggle to adopt NAT due to budget constraints.
