Introduction

A Fixed Dose Drug Combination (FDC) is a pharmaceutical formulation that combines two or more active ingredients in a fixed ratio in a single dosage form. FDCs are designed to improve treatment compliance, particularly in chronic diseases, by reducing pill burden and sometimes enhancing therapeutic efficacy.

Merits of FDCs

  • Improved compliance : Patients take fewer pills, simplifying complex treatment regimens.
    Eg: Anti-Tuberculosis therapy uses standard FDCs to improve adherence over multi-drug regimens.
  • Potential enhanced therapeutic effect : Certain combinations can synergistically improve outcomes when clinically justified.
    Eg: Some cardiovascular FDCs combine antihypertensives to better control blood pressure.
  • Reduction in medication errors : Fixed formulations reduce the risk of patients taking incorrect doses or missing components of multi-drug regimens.
    Eg: Standardized FDCs in chronic HIV therapy ensure patients consistently receive all necessary antiretroviral agents.

Demerits of FDCs

  • Therapeutic irrationality : Many FDCs lack scientific justification and may be ineffective or harmful.
    Eg: Health Ministry banned 16 FDCs in 2026, including dermatological and antibiotic combinations, for being “deemed irrational or unsafe.”
  • Dose inflexibility and adverse reactions : Fixed ratios prevent individual titration and make it difficult to identify the cause of side effects.
  • Public health risk : Irrational antibiotic FDCs contribute to antimicrobial resistance.
    Eg: In 2016, the Ministry invoked Section 26A to ban 344 irrational FDCs based on the Kokate Committee report, where antibiotic cocktails represented a substantial share pertaining to rising AMR in India.
