The Kerala government has announced India’s first dedicated department exclusively for the welfare and care of senior citizens.

About Department for Welfare of Senior Citizens, Kerala

  • Aim: To address the growing social, economic and healthcare challenges arising from Kerala’s rapidly ageing population.
  • Existing Institutional Support: Kerala has already established
    • The State Elderly Commission (2025), a quasi-judicial body for protection of senior citizens’ rights.
    • Community initiatives such as Vayomithram for mobile medical care and Samayaprabha daycare centres.
    • The Kerala Care Palliative Grid integrating government institutions and NGOs for palliative care delivery.

Changes in Kerala’s Demography

  • Rising Elderly Population: Kerala is India’s fastest ageing state, with around 16.5% of its population above 60 years, projected to reach 20.9% by 2031.
  • Increasing Old-Age Dependency Ratio: Kerala’s old-age dependency ratio increased from 19.6% in 2011 to 26.1% in 2021 and is expected to rise to 34.3% by 2031.
    • Old-Age Dependency Ratio (OADR) refers to the number of elderly dependents per 100 working-age persons.
  • Growth in Old-Age Homes: The number of residents in old-age homes increased from 19,149 in 2016-17 to 37,895 in 2024-25, indicating rising elderly-care needs.
  • Higher Rural and Female Ageing: Around 17.5% of Kerala’s rural population is above 60 years, while women significantly outnumber men in the 80+ age group.

Reasons for Faster Ageing of Kerala’s Population

  • Declining Fertility Rate: Kerala’s Total Fertility Rate declined to 1.35 in 2023, far below the replacement level fertility rate of 2.1.
  • Higher Life Expectancy: Improved healthcare facilities increased life expectancy to around 78.4 years for women and 71.9 years for men.
  • Large-Scale Migration: Migration of the working-age population abroad has left many elderly parents living alone in Kerala households.
  • Return Migration of Elderly Workers: Many migrants returning from West Asia after retirement have further increased the elderly population in the state.

Challenges with Kerala’s Ageing Population

  • Growing Healthcare Burden: More than 70% of Kerala’s elderly suffer from chronic illnesses such as diabetes, hypertension, arthritis and cardiovascular diseases.
  • Mental Health and Dementia Concerns: Nearly one-fourth of elderly persons show psychological distress, while dementia prevalence among people above 65 years remains significant.
  • Shortage of Geriatric Care Workforce: Kerala currently has around 120 geriatricians and 600 trained elderly-care nurses, far below projected requirements for 2030.

Significance of a Separate Department for Senior Citizens

  • Focused Institutional Mechanism: The department will provide dedicated administrative focus for elderly welfare, geriatric healthcare and social protection policies.
  • Strengthening the Care Economy: The initiative can promote development of trained caregivers, palliative care services and elderly-support infrastructure in Kerala.
  • Model for Ageing Governance in India: Kerala’s initiative may become a model for other Indian states facing demographic transition and rising elderly populations.
